Purpose of Job
Meets Agency Mission by overseeing the success of the Agency’s
community based programs within Clatsop or Tillamook County. Such
programs include: Nutrition Services, Family Caregiver Support
(FCSP), Senior Peer Counseling, Senior Health Insurance Benefits
Assistance (SHIBA), Options Counseling, Self-empowered Training for
individuals who employ Homecare Workers, and who are people with
disabilities or Seniors (STEPS), the Otago fall prevention program,
and other healthy aging, disease prevention and health promotion
programs.
Essential Functions

Develop and promote new and ongoing program services in
assigned County, including outreach to ensure program success

Oversee program services in assigned County

Ensure assigned staff and program volunteers meet Agency
mission, values and program requirements

Reduce consumer and Agency risk
-
Provide Options Counseling services and Private Admission
Assessments (PAA)
-
Connect family caregivers with resources and services
-
Promote ADRC concept, services and specifically Option
Counseling and Family Caregiver Services
-
Provide excellent customer service in a professional manner
1. Develop and promote new and ongoing program services in
assigned County, including outreach to ensure program success
-
Evaluate needs and identify program opportunities
-
Develop and implement outreach activities to seek out
recipients for all services
-
Educate the community
-
Network with community partners
2. Oversee program services in assigned County
-
Work closely with Community Programs Manager, Services
Managers and other program managers to ensure services within
assigned area are meeting client needs, quality standards,
Federal, State and Agency requirements
-
Recommend changes as needed
-
Screen and process new referrals to the HOPE Senior Peer
Mentor Program, including interviewing and providing program
overview to consumers, conducting home visits, administering
PHQ-9 and GAD-7 screening tools and documenting the visit and
disposition according to program policy and procedure
-
Conduct intake appointments (via phone or home visit) for
the Otago fall prevention program

Coordinate weekly Otago team meetings
-
Act as liaison between NWSDS staff and Otago program staff
on shared consumers
3. Ensure assigned staff and program volunteers meet Agency
mission, values and program requirements

Manage some volunteers directly and others indirectly
through staff
-
Manage, coach and mentor staff and volunteers
-
Train and manage performance, including effective
application of human resource policies
-
Promote two-way communication by engaging others, listening,
providing feedback and expressing appreciation
-
Provide direct services as necessary when staff/volunteers
are not available to ensure functioning of program

Serve as a Mandatory Reporter of suspected abuse of
vulnerable populations as required by policy and regulation

5. Provide Options Counseling services and Private Admission
Assessments (PAA)
-
Determine the need and appropriateness of options counseling
through gathering information from seniors or people with
disabilities, family members, professionals and other support
resources about the individual’s wants, preferences, goals, and
willingness to participate

Explain available options, facilitate decision making,
support and advocate for individual’s preferences and decisions
and develop action plans
-
Facilitate access to services by helping individual and
family initiate contacts for services, assisting with
application process, making calls, following up on progress, and
evaluating situations throughout process, identifying changes
and altering care plan
6. Connect family caregivers with resources and services
-
Identify and assess potential caregiver needs, providing
care planning consultation and intervention in gaining access to services
-
Determine eligibility for Family Caregiver Support Program
services available based on resources and situation
-
Locate services, conduct assessments, monitor services
provided, and work with caregivers to ensure access to services
-
Facilitate family caregiver support groups
-
Approve respite care services and program payments according
to authority guidelines
7. Promote ADRC concept, services and specifically Option
Counseling and Family Caregiver Services
-
Educate self and others of private and public resources
available for populations served
-
Consult with colleagues, community partners and resourcing
electronic and written materials about resources and current
issues and trends
-
Share education obtained by providing updates for ADRC
resource database, building relationships with community
partners, and participating in networking
-
Identify unpaid family caregivers and others for potential
services through outreach with Agency staff, community
organizations, etc., including community presentations
